It is an island governed by<br \/>\nvicious exploitation of the system, crass contempt for the people and<br \/>\ntotal indifference to the pervading misery of the masses. Therefore, it<br \/>\nis not uncommon for the Minister of Education, in his mindless elitism,<br \/>\nand unconcealed disdain for the educational system he administers, to<br \/>\nhave all his children studying in schools overseas. It is also not<br \/>\nunusual for the Minister of Health, in his utter scorn for the health<br \/>\ncare system that he superintends, to travel to Britain, Germany or<br \/>\nAmerican, just for routine medical checkups.<br \/>\nWithout any personal stake in the institutions they administer, they are<br \/>\ncondescendingly uninterested in the decline and decay of Nigerian<br \/>\ninstitutions. The bane of the Nigerian health care delivery system is<br \/>\nthe refusal of the Nigerian power elite and their family members to<br \/>\nutilize Nigerian hospitals. The first step in improving the state of<br \/>\nNigerian hospitals is in putting an end to medical tourism by our rulers<br \/>\nand their family members, and thus, making it imperative that they use<br \/>\nNigerian hospitals.<br \/>\nAs of 2014, overseas medical trips by high ranking government officials<br \/>\ncost the federal government about N250 billion ($1.25 billion).The<br \/>\nJonathan administration tried to reduce medical tourism to its barest<br \/>\nminimum by stopping government officials from seeking medical treatment<br \/>\nabroad, except in cases it can be proven that the needed medical<br \/>\nservices do not exist in the country. Nigerians had reasonably expected<br \/>\nthat President Buhari, in consonant with his promised \u201cchange\u201d, will<br \/>\nreduce medical tourism and its attendant toll on public coffers, and<br \/>\ncorollary, improve the health services with the country.<br \/>\nParadoxically, more than any other Nigerian president, Mohammadu Buhari,<br \/>\nglamorized medical tourism. It received a matchless boost from his<br \/>\nadministration. Some of his medical trips were necessary, as some of his<br \/>\nillnesses demanded medical services not available in the country. But,<br \/>\nat least, one of them, to treat an ear infection, was certainly<br \/>\nsuperfluous. An ear infection is not an exceptionally grievous illness;<br \/>\nit could have been treated in Nigeria. That trip was disconcertingly<br \/>\nreminiscent of the scornful antics of earlier Nigerian administrations,<br \/>\nand their dramatized insensitivity to public sentiments. After more than<br \/>\nfour years of the Buhari presidency, medical tourism continues<br \/>\nunhindered, and the Nigerian healthcare delivery system remains moribund<br \/>\nand in disarray.<br \/>\nIn May 2019, the former Nigerian Minister for Health, Isaac Adewole,<br \/>\nappeared before the Nigeria senate. And the then Senate President,<br \/>\nBukola Saraki, said, \u201cThe senate is concerned with the deteriorating<br \/>\nconditions of hospitals and health facilities across the country.\u201d For,<br \/>\n\u201cdespite the annual budgetary allocation to the health sector, Nigerians<br \/>\nare still dying because of obsolete equipments, poor state of<br \/>\ninfrastructure, lack of generating sets, lack of power, sometimes lack<br \/>\nof diesel, lack of drugs etc \u2026\u201d Ostensibly alarmed about the despicable<br \/>\nstate of the healthcare delivery system in the country, the senators<br \/>\nquestioned the minister; and the scholarly, starchy professor of<br \/>\nmedicine answered their pointed questions. But then, despite the<br \/>\nsupposed solemnity and appropriateness of the legislative exercise, like<br \/>\nmost things Nigerian, it was a charade, just an exercise in hypocrisy.<br \/>\nThe fundamental source of the problems of the Nigerian health system is<br \/>\nobvious, and the senators grilling the Minister for Health knew it. The<br \/>\nhealth system is suffering from neglect. Both at the state and federal<br \/>\nlevels, the system is underfunded. At the African Union Abuja<br \/>\nDeclaration of 2001, African leaders pledged to commit at least 15% of<br \/>\ntheir annual budgets to health. Lamentably, Nigeria has never, for once,<br \/>\nmet this benchmark. It has never allocated more that 6% of its budget to<br \/>\nhealth care. Secondly, due to corruption, a significant percentage of<br \/>\nthe health budget is peculated by government officials and hospital<br \/>\nadministrators.<br \/>\nHealth workers are not motivated because they are inadequately<br \/>\nremunerated, and are sometimes, not paid their salaries for many months.<br \/>\nAnd the federal government is notorious for reneging on its agreements<br \/>\nwith health workers, especially, on remunerations and employee benefits.<br \/>\nNot surprisingly, strikes by medical doctors and other health workers<br \/>\nare prevalent in Nigeria. There is mass exodus of Nigerian doctors and<br \/>\nother health workers, especially, the most qualified, to different<br \/>\ncountries of the world. In addition to reducing the quality of available<br \/>\nmedical workers in the country, it increases the ratio of health workers<br \/>\nto the general population. With this alarming mix of woes, what can be<br \/>\nexpected of Nigerian hospitals?<br \/>\nThey are in a dreadful state. They, especially public hospitals, are<br \/>\nsubstandard and inefficiency: equipments are either unavailable or<br \/>\nobsolete, infrastructure dilapidated, and lacking generating sets,<br \/>\npower, diesel, and drugs. Unmotivated and sometimes, deliberately<br \/>\noperating below acceptably medical and ethical standards, the doctors<br \/>\nand nurses that run them are mean-spirited, bad-mannered and cold<br \/>\nhearted. The hospitals are overcrowded, teeming with patients suffering<br \/>\nand dying from variegated diseases, including readily preventable and<br \/>\ntreatable ones. Not surprisingly, the number of unnecessary and<br \/>\navoidable deaths in our hospitals boggles the mind.\u00a0 Nigeria has one of<br \/>\nthe highest pregnancy-related deaths in the world. With 2% of the<br \/>\nworld\u2019s population, it accounts for 11% of the world\u2019s maternal<br \/>\nmortality and 12% of the world\u2019s under-five mortality.<br \/>\nThese problems will continue to fester until those entrusted with the<br \/>\nresponsibility of solving them, the political elite, are forced to<br \/>\nexperience firsthand the horrors of our hospitals. Until medical tourism<br \/>\nis severely curtailed, and the Nigerian ruling elite are forced to<br \/>\nexperience firsthand the awfulness of the Nigerian health system, our<br \/>\nrulers will not genuinely address the troubles besetting the Nigerian<br \/>\nhealth sector.<br \/>\nRefreshingly, in a recent event at the Alex Ekwueme Teaching Hospital,<br \/>\nAbakaliki, President Buhari, speaking through a representative, demanded<br \/>\nan end to medical tourism because the country can no longer afford it.<br \/>\nNigerians were both baffled and titillated by this demand from the<br \/>\nUbermench of medical tourism. We hope that the statement was dictated by<br \/>\na genuine change of heart towards medical tourism, and that Mr.<br \/>\nPresident will practice what he is beginning to preach.
